Grove Street Residence:


Grove Street is a co-ed specialized residential treatment program serving adolescents referred through the Department of Mental Health (DMH) and The Department of Children and Families (DCF). This program utilizes Dialectical Behavior Therapy (DBT) and Wraparound as the treatment models.


 



Ives Program:


The Ives Program is a comprehensive staff-secure treatment residence for adolescent males and females referred through the Department of Mental Health and the Department of Children and Families. This program utilizes IMR (Illness Management and Recovery) and Wraparound as the treatment models.



School House Program:


This DMH-funded program is a comprehensive treatment and staff-secure residence for nine boys and girls ages 8 to 12 years old with a history of emotional and behavioral difficulties, as well as a history of psychiatric hospitalizations. This program utilizes Wraparound and experiential learning as the treatment models.


 



Northboro Community Residence:


This residence provides a therapeutic living environment for young men and women between the ages of 18 and 22+ with mental health challenges. The program goal is to help young adults referred by the Department of Mental Health to develop independent living skills, healthy relationships, personal independence, and the skills to manage their illnesses. 




The program has two on-site clinicians and vocational day programming. Both DBT and IMR are evidence based practices that are used at NCR to help individuals become more self-sufficient. The goal is to teach the skills necessary for the young adults to live independently.

Responsibilities


Responsible for physical assessments, maintenance of health records and planning and implementing educational groups with individuals served and staff related to each individual’s health and disease processes.



 Assist staff with the implementation of the Medication Administration Program (MAP) and provide training to staff to meet the regulations. Supports and actively participates in agency-wide wellness initiatives. Assist staff with appointment scheduling, follow-up with physicians, and hospital discharge planning. Complete required documentation in a professional and timely manner.




 Eligible candidates must have a solid medical-surgical background due to many of our individuals having co-occurring diagnoses.


 



Qualifications


Current Registered Nurse License (RN) with experience in the treatment of serious emotional and mental health issues, knowledge of psychotropic medication, psychosocial evaluations and ability to work effectively among various programs and with community service providers. Good written and verbal communication skills and ability to collaborate with multi-disciplinary team members required. 




Valid driver’s license and reliable and insured vehicle for travel between programs and providing transportation is required. Position requires the ability to work flexible hours with some early evening hours included.
